Clinical Pharmacy Specialist at Providence Oregon Regional Shared Services
The Infectious Disease Clinical Pharmacy Specialist assists the Infectious Disease physicians, Providence Medical Group, the hospital-based pharmacotherapy clinics, and care teams across the region in the optimization of clinical pharmacy services in infectious diseases. Responsibilities include promotion of evidence-based, safe and effective medication therapy, leadership and oversight of antimicrobial stewardship and infectious diseases initiatives, and standardization of practices across the continuum of care.
Required qualifications:

Graduate from an accredited College/School of Pharmacy.

Completion of PGY-1 Residency training -or-
-
PGY-1: Equivalent experience.

PGY-2: 3 years experience in infectious diseases practice.

Completion of PGY-2 Residency training in infectious diseases -or-
-
PGY-1: Equivalent experience.

PGY-2: 3 years experience in infectious diseases practice.

Oregon Pharmacist License upon hire
-
National Certification from Board of Pharmacy Specialties within 2 years of hire
Preferred qualifications:

Additional training in infectious diseases (e.g., MAD ID Antimicrobial Stewardship Training, SIDP Antimicrobial Stewardship Certificate).

Experience in HIV pharmacotherapy management
