Description:
Johns Hopkins Home Care Group is seeking a motivated individual to assume a clinical position within home infusion as an Infectious Diseases (ID)/Outpatient Parenteral Antimicrobial Therapy (OPAT) Clinical Pharmacy Specialist. The individual will work with an interdisciplinary team to provide clinical management and oversight for OPAT services, including supporting transitions of care services from acute care to home infusion and skilled nursing facilities. The individual will develop and lead quality improvement projects for OPAT services, support antimicrobial stewardship initiatives, and have the opportunity to participate in research activities. The individual will work closely with physicians, nurses, and pharmacists across the continuum of care to ensure safe, timely, and effective OPAT treatment and monitoring. The individual will serve as a preceptor for student pharmacists and pharmacy residents of ASHP accredited PGY1 and PGY2 residency programs. A PharmD degree is required. PGY1 Pharmacy Practice residency training, and PGY2 Infectious Diseases residency training (or equivalent experience) is required. Home infusion experience is preferred, but not required.
